Bid Description:
The Department of Vermont Health Access is seeking a qualified bidder to serve as a Quality Improvement (QI) Practice Facilitator Coordinator (QI Facilitator Coordinator) supporting other State contracted QI facilitators and their activities in primary care practices, women’s health practices and providers, integrated care teams within communities and specialty addictions, and mental health programs under the Blueprint for Health (Blueprint) program. The Blueprint program is a comprehensive delivery system reform program that has developed payment reforms, a health information technology infrastructure, and an evaluation framework to support the development of Patient-Centered Medical Homes, regional Community Health Teams (CHTs), and the All-Payer Model. The Blueprint’s network of QI facilitators is a crucial part of the State’s health reform efforts.
